Job Information | AW Health Care has an immediate full-time opening for a Licensed Practical Nurse (LPN), Case Manager, who will assume responsibility for assessments, care coordination, advocacy, and related case management activities for an assigned group of clients. Inherent in the position is the assessment of patient needs in conjunction with the client and/or family, physician, and ancillary health care providers. MO Licensed Practical Nurse (LPN),experience in in-home assessments of clients, experience establishing & monitoring client’s care plans, and reliable transportation required.
