What is the GUIDE Program?
The Guiding an Improved Dementia Experience (GUIDE) Model is an eight-year, voluntary initiative launched by CMS on July 1, 2024. Its mission: deliver comprehensive, coordinated dementia care to Medicare beneficiaries while supporting unpaid caregivers. It does this through care management, caregiver education, and respite services via participating Dementia Care Programs (DCPs).
