AW Health Care has an immediate full-time opening for a Field Care Coordinator in our Maryville, Illinois office to assist with training and supervision of caregivers, client/employee advocacy, and related care management activities to ensure overall satisfaction of clients and caregivers. High School Diploma/GED, Certified Nurse Assistant (CNA), and 2 years of Home Care experience is required. 1 year of supervisory experience is preferred.
The Field Care Coordinator will:
- Evaluate and train staff in the field to ensure competency of the employees for Home Care
- Assist with Caregiver training (Orientation, Skills Lab, and annual In-Services)
- Facilitate On-the-Job Training as needed
- Provide on-going education, supervision, and training of the Caregivers in the field
- Identify high risk factors, barriers to goals, and strengths that will assist the client and caregivers in achieving goals
- Assist with the assignment and scheduling of field staff to provide care and services for Home Care
- Rotate on-call responsibilities with other members of management
- Provide support for on-call staff, evenings and weekends, as needed
- Cover shifts, in the event of staffing shortage, with direction of management
- Assist with office-related tasks, as well as ensuring accurate opening packets are available with an adequate supply for each week
